Our Health Resource Center - offered in affiliation with St. Joseph Heritage Medical Group - offers an on-site, self-service Health Resource Center that provides patients with:
- Information on health and mental health topics relevant to adults, parents and teenagers.
- Forms and applications for Advance Directives, Medi-Cal, Access transportation, utility assistance and more.
- Computer access enabling individuals to print health information, watch surgical videos and participate in tutorials and health risk assessments.
- Brochures in Spanish and other languages, which can also be found online by clicking here. You may also visit the St. Joseph Hospital Resource Center or Burlew Medical Library for additional information.
