The Center for Health Promotion offers a Care Coordinator to assist patients in accessing the resources they need to maintain their health and well being. The Coordinator provide:
- Emotional support to patients and loved ones when adjusting to a medical condition
- Referrals to behavioral health services, counseling and psychotherapy services
The Care Coordinator also provides information and referrals for:
- Food
- Clothing
- Housing
- Prescription assistance
- Parenting classes
- Substance abuse treatment
- Financial assistance
- Transportation
- Legal assistance
