During Open Enrollment
- Write the physician’s name and health plan ID number on your health insurance enrollment form. Click here for a listing of physician ID numbers.

How to change Primary Care Physicians (PCP) to join St. Joseph Hospital Affiliated Physicians
- Contact the Member Services or Customer Services Department listed on your insurance card.
- Give them the name of the new Primary Care Physician you have selected and specify you want to join the St. Joseph Hospital network.

- Confirm your selection with your health plan.
