Giving your family the quality care and dedication provided by the doctors of St. Joseph Hospital Affiliated Physicians (SJHAP) is easy as 1-2-3.  If you belong to one of our accepted health plans, just follow the steps below.

      1.   Call us at (866) 888-9641

• We can tell you if your current doctor can be accessed through St. Joseph Hospital Affiliated Physicians (SJHAP)
• We can help you choose the physician that's best for you and your family.
• We'll give you your Health Plan phone number and the Provider ID Number to choose your physician.
• We'll answer any questions you may have.

      2.   Call the Member Services Department at your health plan or contact your Human   
             Resources Department at work

• Give them your physician's Provider ID Number
• Confirm with them that you are choosing your physician through St. Joseph Hospital Affiliated Physicians.

      3.   Rest assured. You are part of the St. Joseph family.
